Employee injured in fall when storage rack collapses

At approximately 3:00 p.m. on April 8, 2000, Employee #1 was taking inventory in a warehouse. He was climbing along the top of a 12 ft high storage rack system filled with heavy tile when he received a phone call. A forklift operator drove to where Employee #1 was working to lower him, but the forklift struck the top crossbrace of the storage rack, dislodging the tile. The section of the storage rack from which Employee #1 was working collapsed, and the tile and rack fell on top of him. He sustained multiple abrasions and a fractured ankle and was transported to U.C.-Davis Medical Center in Sacramento.
