Investigation Summary

Investigation Nr: 202549515
Event: 07/09/2008
Employee's Finger Is Crushed in Folder Gluer

On July 9, 2008, Employee #1, a gluer, assistant, finisher, and stacker, was cleaning the glue transfer belts at the cardboard box pick up point of a folder gluer, BOBST Post 3115 Matic. The folder gluer was a cardboard box gluing and folding machine. The transfer belts were cleaned, when necessary, at the pick up point to remove ink residue left by previous board transfer. Employee #1 had a towel in his right hand wiping the two glue transfer belts at the pick up point, while they were rotating. His towel and right hand were inadvertently caught and pulled in between the two rotating belts. Employee #1 sustained a crushing injury to his right ring finger. He was transported to Concentra Occupational Medical Center in Anaheim, California. Then, Employee #1 was transferred to the University of California Medical Center UCI in Irvine, California, where his right ring finger was amputated.
