Investigation Summary

Employee #1, an inmate, was working in the woodwork shop at the prison where the workers were making wooden toys for children. He reached under a table saw to unhook a vacuum hose to use it as a hand vacuum. At the same time, another inmate was running the saw and didn't see him. As Employee #1 grabbed the hose, two fingers on his right hand were amputated. The saw was properly guarded and enclosed on all sides. The only access was from the bottom, where the local exhaust system was attached.
