Investigation Summary

Employee #1, an electrician, was installing seven air conditioning units on the roof of a 10-year-old commercial building. He was working on wiring on one of the units but needed parts that were in front of another unit. Employee #1 walked approximately 25 ft across the roof to pick up the parts and was walking back when he apparently stumbled on the ledge of a convex Plexiglas skylight. The skylight shattered when he landed on it and he fell 30 ft to the concrete floor. Employee #1 was immediately transported to the hospital, where he died of massive head injuries. There were no witnesses to this accident.
