EMPLOYEE SUFFERS AMPUTATION OF FINGER

At approximately 1:35 p.m. on May 21, 1998, Employee #1suffered the amputation of the left pinkie finger at the first joint. Employee #1 was working as a production inspector in a plant that manufacturing unfinished wooden cabinet doors. In this operation the operator places the door on a conveyor; the door passes through the cutting heads/sander section, and is off-loaded at the end of the conveyor. The conveyor is made of metal segments, with each segment forming a rectangle approximately 2.75 in. long. During a break in the operation Employee #1 turned to update the job ticket form. As Employee #1 turned back to the machine she lost her balance. Employee #1 tried to balance herself by placing her left hand on the machine. Her left pinkie finger entered into the opening between the conveyor segments. The opening is created as the conveyor reaches the end and begins to travel downward. The opening is 0.5 in. wide by 2.75 in. long by 0.5 in. deep. Employee #1's finger was sheared off as the conveyor met the off-feed table. Employee #1 was transported to the hospital for treatment of her injury.
