Investigation Summary
At 5:45 a.m. on April 15, 1998, Employee #1 was working in a refrigerated warehouse, attempting to unjam a palletizer machine. He had not de-energized the unit and while restacking crates he inadvertently activated a limit switch, causing the pusher to pin him against the frame of the machine. He sustained fractured ribs and internal injuries, for which he was hospitalized for four days.
