Employee's finger amputated while operating a fork lift

Employee #1 was operating a forklift and he approached a closed door driving in reverse. He pulled a rope to open the door and passed through. He then grabbed the rope while sitting in the operator's position of the forklift to close the door. He drove the forklift slowly in reverse while holding onto the rope ensuring the forks had cleared the door. As he drove in reverse the rope was getting shorter and his right arm was rising to the upper edge of the forklifts overhead guard. When his hand reached the guard, between the pulling of the rope and the edge of the guard, his finger was pulled and amputated between the first and second joint.
