Employee injured when caught by moving rotary valve

At 12:25 p.m. on May 18, 2001, Employee #1 reached into the top of a running open gatelock, Number 133, to retrieve a dropped tool. The access cover had been removed. Employee #1 reached into the interior of the gatelock and two fingers of Employee #1's right hand came into contact with the moving rotary valve unit. Employee #1 suffered the amputation of a finger and was hospitalized.
