Employee injured when he losses his balance and falls

Employee #1 was working approximately 3 to 3.83 ft down inside an air conditioning unit. The air conditioning unit was 8 ft high by 9.83 ft long by 7 ft wide. When he was finished, he stood up and placed both hands behind him, resting his palms on the top of a steel plate. He then pushed down on his palms lifting himself up onto the plate in the sitting position. He lost his balance and fell backwards, striking the back of this head on a steel beam. He continued to fall landing on the concrete slab 8 ft below. Employee #1 sustained multiple head and neck injuries and was hospitalized.
