Investigation Summary
On August 12, 2000, Employee #1 suffered first and second degree burns to his body when he fell into a hot liquid while attempting to close a 10-inch valve with a 2-inch plug to make repairs with a coworker. He was using a pipe wrench with an extension bar. The top corner piece of the plug broke off. The wrench slipped, and Employee #1 fell off the grating section he was standing on and into the overflow hot process juice. This was in a containment area. Employee #1 was hospitalized with first and second degree burns to his body.
