Investigation Summary

Investigation Nr: 201484094
Event: 03/19/2001
Employee's Thumb Amputated by Table Saw

Employee #1, a carpenter, was assigned to the JET table saw. The foreman showed him the task he was to perform but did not instruct him on the hazards of the saw. When he started the task of cutting 2 inch strips of plywood off of a 4-foot by 8-foot sheet, the guard over the blade was missing. Employee #1 was wearing cotton gloves. Within the first 5 to 10 minutes of operating the saw, his glove got caught on the saw blade which pulled his left hand into the saw as he guided the plywood forward. Employee #1 stated that the wood could not freely travel forward because there was another sheet of plywood on the table and the pieces he was cutting would strike the extra sheet of plywood. Employee #1 had to give the cut plywood a push to get it past the extra sheet. Employee #1 amputated his thumb in the machine and was hospitalized.
