Investigation Summary
On September 5, 2001, Employee #1 was standing on the hood of a 1980 Caterpillar D7 track-type tractor , trying to determine how best to install a new radiator. For some reason, he fell 6 ft off the hood to the ground, dislocating two vertebrae. Employee #1 was killed.
