Employee's Finger Is Amputated in Valve

A 38-year-old maintenance worker was troubleshooting a vacuum leak at a Safeway manufacturing facility. A coworker, who was in training, asked about an "open" sign on a valve. The maintenance worker pointed to the valve, which was 50 millimeters above his head and about 600 millimeters away. As he pointed to the valve, the condenser receiver cycled, and the actuating ball valve closed on his finger and amputated it. The maintenance worker was hospitalized for his injuries.
