Investigation Summary
On April 12, 1999, an employee for a concrete product manufacturer was injured. The employee reached into a Columbia Model 50 concrete mold machine to reinsert a mold ring that had dropped. The moveable table restarted and rose vertically and pinned the employee's hands against the mold units. The employee received crushing injuries to both hands and a partial amputation to his right-middle finger.
