Employee drives forklift off an elevation

At approximately 9:30 a.m. on April 4, 1999, Employee #1 was driving a forklift down a warehouse aisle in reverse and failed to stop at the end of the aisle. He drove through a closed rail door and fell approximately 5 ft, landing on railroad tracks. He fractured both ankles and was hospitalized.
