Fingers amputated when caught between forklift mast and van

On September 19, 1998, an employee was injured while operating a forklift. The employee, a forklift mechanic for a structural steel manufacturer, was attempting to park a Yale Forklift between the building and a van. The open sea van door was being used as a tool crib. The employee caught his left index and middle fingers between the left forward mast of the forklift and the van door. The tips of his left index and middle fingers were amputated.
