Employee Killed In Fall From Conveyor Beam

Employee #1 was instructed to clean the overhead lights in a cooler after his lunch. Employee #1 told a coworker at the start of the shift he was not feeling well and should have stayed home. After lunch Employee #1 cleaned the lights using a scissor lift. Employee #1 climbed out of the lift onto a 10-ft-11in.-high beam and straddled the beam. Employee #1 fell off the beam onto the concrete floor striking his head and back. Employee #1 was not wearing a harness or lanyard when he climbed out of the basket. Employee #1 was hospitalized for fractures.
