Employee injured in fall from roof

On April 23, 1999, Employee #1 was standing on a roof of a house where the roof was being repaired. A 2 by 4 board was nailed on the roof for the material to be stacked up against to keep it from sliding off the roof. The first bale of shingles was placed flat on the roof behind the 2 by 4 board. The second bale was placed flat on the roof next to the first, working its way up to the peak of the roof. By time the stack of shingles reached 15 bundles, it was too much weight for the 2 by 4 board to hold back and tore out the 2 by 4 board allowing the bundles of shingles to slide down the roof. The material struck Employee #1 and knocked him off the roof. He fell 14 ft to the ground, landed on his feet, and sustained fractures.
