Investigation Summary
On May 24, 1999, Employee #1 was loading a conveyor with Styrofoam trays for packaging chicken. He reached under the guard for the conveyor's chain and sprockets drive to unblock a cart loaded with Styrofoam trays. The bottom of the 15-in. by 9-in. by 18-in.-high guard was open allowing Employee #1's left thumb to be caught by the machine and amputated. Employee #1 was taken to the hospital.
