Investigation Summary

Investigation Nr: 170618664
Event: 07/03/1998
Employee injured in fall with ladder

On July 3, 1998, Employee #1, a repairman for the Chinet Company, was working at its paper plate manufacturing facility in Sacramento, CA. He and a coworker placed a 9 ft ladder against the 8 ft 6 in. tall wall of the lab building. The coworker climbed onto the roof using a vertical pipe as a handhold, and Employee #1 began to do the same. At a height where his waist was near the top of the lab wall, Employee #1 started to hand a tool to his coworker when the bottom of the ladder broke free. It slid down the lab wall, leaving black marks most of the way down. Employee #1 became entangled in the ladder and fell with it to the floor. He suffered multiple fractures of his right ankle and was transported to Kaiser Hospital in South Sacramento, CA, where he was admitted for treatment.
