Investigation Summary
On November 16, 1998, Employee #1 was opening the cap of an aboveground gasoline tank. He was holding the delivery hose nozzle in his right hand when the nozzle and the ferrule (coupling) came off the hose, soaking his clothes with gasoline. Employee #1 turned off the pump at the truck, rinsed off himself and his clothes with a water hose, and then began to clean up the gasoline spill. He also contacted his offsite manager about the spill, asking the manager to bring him a change of clothes. Employee rinsed off two more times during the cleanup, but could not change clothes for almost two hours due to the distance between his home and the work site. On returning to his home base, Employee #1 dropped off the truck and sought medical care, where he was treated and released. He then took the damaged hose to a supplier for repair. On November 19, 1998, Employee #1 was hospitalized for treatment of chemical burns at the UC-Davis Medical Center Burn Unit; he was released two days later and returned to work on November 25, 1998, without further restrictions. When Cal/OSHA opened its inspection on December 8, 1998, the hose involved had been repaired, but not pressure-tested. The coupling manufacturer tested it and found that the wrong-sized ferrule had been installed during an earlier repair.
