Employee Crushed and Killed by Lift Truck

Employee #1, a roofer, was directing the driver of a materials high-lift truck who was backing into a garage of a private residence. For an unknown reason Employee #1 stepped behind the truck as it was backing up to the wall of the garage. The driver stated that Employee #1 disappeared from his line-of-sight. The driver heard Employee #1 yell, and saw him run from the back of the truck to the front of the truck and collapse. Apparently Employee #1 was crushed between the truck and the wall of the garage.
