Investigation Summary
On November 22, 2010, Employee #1 was working as a millwright at a sawmill. Employee #1 was constructing a wall to form a barrier between two operations. While he was placing a header in a door frame, Employee #1 stepped down from a ladder to get a shim, which he planned to place under the header. On his was back up the ladder, after retrieving the shim, Employee #1 accidentally bumped the wall frame. This caused the header to fall, landing on Employee #1's left index finger, which was resting on the top cap of the ladder. Employee #1 was taken to a nearby medical facility, where he was hospitalized and underwent surgery to repair a fracture received on his left index finger.
