On July 7, 2010, Employee #1, a mechanic of Steve Willis Trucking and Lodging, LLC, was standing on a ladder making repairs to a 960 Caterpillar loader. Employee #1 fell from the ladder and was hospitalized for treatment of a fractured pelvis.
