Investigation Summary
On May 20, 2010, Employee #1, a truck driver of Boyd Transportation Inc., was covering his load of fuel shaving with a tarp when he slipped and fell. The rope on the tarp broke causing Employee #1 to lose his balance. As Employee #1 was falling, his ankle rolled under him, placing all of his weight on it. Employee #1 had surgery to place two pins in his ankle.
