Investigation Summary
On November 7, 2007 Employee # 1, a temporary water bottler, was working as a Sidel operator (packaging operator position). Employee #1 was assigned to clean up at a conveyor system during a lull in packaging operations. Employee #1 was using a cloth rag to wipe down the conveyor system. Employee #1 wrapped the rag around his left hand. Employee #1 did not stop or lockout the conveyor system. The conveyor system was running when Employee #1 wiped down the underside of the conveyor at a roller. Employee # 1's hand was pulled into the space between the roller and conveyor belt resulting in an injured hand.
