Investigation Summary
On June 5, 2007, an employee was working for rice farmer as a farm hand. The employee was placing air in the tire of a trailer, when the split rim came off the wheel. The employee's hand was struck by the metal piece of the split rim. The employee was hospitalized and treated for a fractured left pinky finger.
