Investigation Summary
Employee #1, working as a carpenter for a remodeling contractor, was ripping a board on a Bosh 10-inch table saw. He was preparing a template but used a poor piece of wood to cut out the template. A piece of the board broke off as he was running it through the saw and it struck him in the left eye. He was wearing eye protection but the eye protection shattered on impact. He was hospitalized with his injury.
