Investigation Summary
On November 8, 2006, Employee #1 was working in the finishing department of a fiber glass manufacturing plant. When he reached to pull a piece of scrap from the main conveyor line, his glove became caught between the edge of the belt and a pulley. The tips of Employee #1's left middle and ring fingers were amputated.
