Investigation Summary

Investigation Nr: 170870620
Event: 02/05/2002
Employee's right finger amputated by a saw

On February 5, 2002, Employee #1 of Pool Master Inc., was cutting half-inch polyethylene foam using an electric Eastman Cardinal Round Knife Machine. The circular knife, with a 5-in. blade, runs between two metal guides (to maintain straight cut) and is suppose to be pushed by use of an 18-in. handle. Employee was aligning a piece of foam with both hands when he leaned over and pushed the machine forward into his finger. He was taken by the supervisor to the company's urgent care provider and later transferred to UC Davis Medical Center in Sacramento. Employee's right index finger was amputated at the first joint but was reattached at the hospital, though reattachment was not successful.
