Investigation Summary
An employee was tightening the bolts on the top of a mobile Cone Crushing Machine. The top of the frame of this machine was just over six ft above the ground. Prior to starting the assigned task, the employee decided that he had to use the rest room. Instead of using the catwalk and attached ladder to safely descend, he attempted to climb down the equipment via the frame. While attempting this maneuver, he felt his feet start to slip. He tried to jump from the frame rather than just fall to the ground. He landed on a metal die that was on a pallet at ground level and fractured his leg. He was hospitalized with his injuries.
