Investigation Summary
On September 27, 2002, Employee #1, a temporary worker, was removing a vinyl window frame from a welding unit. Employee #1's right hand became entangled in the revolving shaft assembly of the unit's take-off conveyor. Employee #1 sustained multiple fractures to his right fingers and wrist, and significant tissue damage. Employee #1 was hospitalized and was released from the hospital on September 30, 2002.
