Employee injured after falling from ladder

On September 25, 2001, an employee of Intech Mechanical Co., Inc. was climbing up a fiberglass ladder to accomplish layout of gas measurements at roof level. As he was stepping off on to the roof with his right foot, the ladder began sliding to his left and he fell approximately 11 ft to the asphalt ground. He suffered fractures but was not hospitalized.
