Investigation Summary
On July 19, 2001, Employee #1 was working as an electrician for an electrical contractor in El Dorado Hills, CA. He was repairing an electrical problem associated with ceiling lights. He was on an 8-foot fiberglass A-frame ladder, looking up inside the ceiling at power packs that were part of the electrical system for lights. He needed the serial numbers of the power packs so that he could order more. The ceiling was 9 feet high, and Employee #1 was standing on the third step from the top. He decided to go back down the ladder to get a flashlight to see the power packs better. On the way down, he turned a little, and when he did, the ladder fell toward the wall. Employee #1 fell to the ground a distance of approximately 5 to 6 feet. He sustained a compound fracture in his leg above his ankle. He was hospitalized.
