Investigation Summary
On September 1, 1999, Employee #1, a carpenter for a general building contractor, was beginning to ascend a ladder to carry a saw up to the roof and hand it to a coworker. The ladder's feet were resting on a dirt slope that went from the base of the ladder downward to the foundation of the house; the top of the ladder was resting against the eave of the roof, which was approximately 6 ft above the ladder's feet. As Employee #1 mounted the first rung, the ladder pivoted out from under him. He fell approximately 1 ft to the ground, then rolled down the slope to the base of the foundation, fracturing his left femur. He was transported by private vehicle to Sierra Memorial Hospital in Grass Valley (CA) and then transferred by ambulance to Sutter Roseville Medical Center in Roseville. He underwent surgery and was released on September 6, 1999.
