Investigation Summary
At approximately 6:55 a.m. on April 2, 1998, Employee #1 and coworkers were unloading a truck at a Rite Aid Drugstore in Sacramento, CA. They were using a belt conveyor with powered rollers at sections going around the corners to move merchandise from the basement to various floor levels. A section of the powered rollers between the first and second floors was broken, requiring a worker to be stationed at the corner of the stairs to push the boxes along. To access this corner area, Employee #1 was riding the conveyor from the first floor when her foot became caught and was pulled down between the moving conveyor belt and the roller at the end. Fire department paramedics had to remove a section of the conveyor to free her leg before transporting her by ambulance to the University of California Medical Center in Sacramento for treatment of a fractured foot and severe lacerations. Employee #1 chose to ride the conveyor to the corner area rather than using the 5 to 6 ft tall ladder that stood on a stair landing beneath the conveyor and that led, without hand holds, to a 6 ft high platform.
