Investigation Summary
Employee #1 was switching the front tires on a front-end loader. Employee #1 had been using a forklift to move the tires and had positioned the right tire on the hub. Employee #1 decided to "man handle" the tire onto the left hub. Employee #1 had trouble moving the tire because it had metal studs that were digging into the asphalt. Employee #1 lost control of the tire and it fell towards him. The impact of the tire buckled Employee #1's legs, dislocating his knee. Employee #1 then grabbed the tire so he would not hit his head on the pavement. The tire fell on his legs, pinning them, with his upper body at ninety degrees in a sitting position. In addition to a dislocated knee Employee #1 received a 38 percent compression fracture to vertebrae L1 in the lower back.
