Investigation Summary
Two employees were cleaning the area underneath an aggregate conveyor in a rock quarry. They were using a high-pressure water hose which was attached to a water truck. Employee #1, the conveyor operator, and a coworker, the water truck driver, were working where the aggregate conveyor made the transition from water to land. The coworker fell into the water and Employee #1 drowned while trying to assist him. Neither employee was wearing a lifejacket at the time of the accident.
