Employee's fingers crushed in machine

While forming a long strip of metal on a press brake, Employee #1, the operator, used his right hand to hold the part near the die. He did not use the tongs that had been provided. The press crushed the ends of three fingers. He was hospitalized and subsequently lost the ends of the three lower fingers on his right hand.
