Investigation Summary

At approximately 11:20 a.m. on October 27, 2000, Employee #1, a teacher tripped over another student while demonstrating a particular basketball maneuver. When he tripped, he fell directly on his right elbow and broke it. His daughter was called, who came to the school and immediately took her father to the hospital. The employee was in the hospital for two days. While in the hospital he had two different operations on his elbow. Three days after being released from the hospital the employee died in his home from cardiopulmonary arrest due to pulmonary emboli.
