Employee's hand injured in chain track of tire machine

Employee #1, age 20, was building tires. It was close to break time and as he reached over the machine to get his water jug, he knocked a tape measure into the machine. The tape measure fell inside the chain track, which was missing a dust cover on one section. Employee #1 thought the machine had finished its cycle, so he reached into the chain area to retrieve the tape measure. The machine cycled, and his arm became caught in the chain track. Employee #1 sustained lacerations to his hand and arm, and he suffered permanent damage to his hand.
