Investigation Summary
Employee #1 and a coworker were checking the diversion valves as part of a quarterly maintenance inspection in the processing area. The valves had to be partially disassembled in an area that was congested with piping. There was a miscommunication between the two workers, and Employee #1 was unprepared when the coworker let down a part to him. His finger was smashed and had to be amputated above the first knuckle.
