Investigation Summary
At approximately 8:30 p.m. on November 14, 1996, Employee #1, of Manpower Temporary Services, was working for Exel Logistics, a subcontractor of Hewlett Packard. He was standing on a pallet mounted on the forks of an order picker, pulling orders from storage racks. Employee #1 was tied off with a harness and lanyard, but after working for some time, he had severely twisted his lanyard and disconnected it. He was standing at the front edge of the elevated pallet, untangling his lanyard, when the pallet slid off the forks. Employee #1 and the pallet fell 13 ft to the floor; he suffered a compound fracture of his right arm, minor back injuries, and numerous contusions. He was given first aid on the scene and then taken by ambulance to Sutter Roseville Hospital, where he was treated for three days. The pallet had been inappropriate for the task because it did not fit into the pallet clamping device on the front of the order picker and it did not have any bottom slats.
