Investigation Summary
At approximately 1:00 p.m. on August 9, 1996, Employee #1, a clerk, was standing on a stepladder, arranging merchandise in an overhead display. An item slipped from the display rack and, as he reached for it, he fell about 5 ft to the floor. Employee #1 suffered a serious fracture of the T12 vertebra, and was hospitalized for three days.
