Three Employees Injured after Fall from Scaffold

On January 6, 2005, Employees #1 through #3, part of a four-person, were cutting holes in the exterior of a church building and spraying insulation inside the front wall. The employees were using a rolling scaffold that was two sections high, approximately 10 ft in height from the top platform to the ground. In order to reach the higher sections of the wall, extension ladders had been placed on the top platform and tied to the scaffold frame. At the time of the incident, Employee #1 and Employee #2 were working from the ladders approximately 20 ft from ground level. Employee #3 was standing on the top scaffold platform. The scaffold pushed away from the wall and fell backward. Employee #1 fell directly to ground and suffered broken bones around his right eye and a fractured spine. Employees #2 and #3 fell to ground from the scaffold and suffered contusions. Employee #1 was hospitalized. The employees had not been trained in the safe erection and use of rolling scaffolds and there was no qualified person present to supervise the scaffold erection.
