Investigation Summary
At approximately 7:30 p.m. on March 31st, 2005, Employee #1 was watching Employee #2 repair a pump attached to asphalt silo tank Number 6. After closing all valves, Employee #2 proceeded to remove the rear cover plate from the pump when hot asphalt sprayed onto Both Employees #1 and #2. Employee #1 suffered third-degree burns and Employee #2 suffered second-degree burns. Both employees were hospitalized and treated for their injuries.
