Investigation Summary
At approximately 3:50 a.m. on August 25, 2006, Employee #1 was standing at a highway construction site located at State Highway 99 and S. Clovis Avenue in Fowler, California. It was the night shift and there was heavy fog. A construction dump truck backed into Employee #1. Employee #1 was sent to the hospital with a fractured leg. Later investigation determined that the backup alarm of the vehicle was not operational due to loose or frayed wire.
