Employee's Finger Amputated in Unguarded Conveyor

On October 19, 2005, Employee #1 was using a towel to brush debris from the moving belt on an energized conveyor. The towel became caught in the unguarded nip point created by a chain and sprocket. Her fingers were pulled in and her index finger was amputated to the first joint, and the middle finger was lacerated. Employee #1 was hospitalized.
