Investigation Summary
On August 1, 2005, Employee #1 was operating a Delta 10-in. table saw set on the bed of his truck. The saw blade was guarded and installed with an anti-kickback device. He was using a push stick when a dog distracted him when it ran between his legs. Employee #1 lost control of the work and sustained amputation injuries to three left hand fingers. Employee #1 was hospitalized.
