Investigation Summary
On September 27, 2004, Employee #1 was installing roof sheathing on the edge of a two-story house when he fell 18 ft to the ground. He sustained multiple facial fractures, an eye orbit fracture, and blunt head trauma with intracranial abnormalities, multiple skull and pelvic fractures, five rib fractures, and a fractured left wrist. Employee #1 was hospitalized and died 23 days later. He was not using any form of fall protection at the time of the accident.
